Therapeutic Action
Non-opioid analgesic, non-steroidal anti-inflammatory class (NSAID)
Indications
Short-term management of moderate to severe post-operative pain in adults and children over 2 years of age.
Instructions for use
Must be given by IV route intraoperatively or IM route after the intervention, as a single dose.
It may be continued for up to 48 hours when short-term NSAID treatment is necessary, and oral administration is not possible.
Prefer ibuprofen when oral administration is possible.
Remark:
For periopertaive use, IV Ketorolac is more suitable than IM diclofenac, as it does not require dilution or buffering with sodium bicarbonate, facilitating preparation and reducing associated risks. Ketorolac can be administered by IV route without additional preparation.
Precautions for Use
Do not administer by epidural or spinal route.
Not recommended in children under 2 years of age.
Contraindicated in patients with:
- coagulopathy, major haemorrhage, ongoing bleeding, traumatic brain injury, and spinal cord injury
- history of myocardial infarction
- pre-existing or de novo renal failure, risk of acute kidney injury (hypovolaemia, shock)
- ongoing peptic ulcer
- presence of an NG tube and not being fed
- during pregnancy
- patients on aspirin for cardiovascular prevention
Do not combine with:
- Other NSAIDs (e.g., aspirin, ibuprofen, or diclofenac)
- Anticoagulants (e.g., warfarin, heparin, or clopidogrel)
- Probenecid
- Lithium
Do not mix in small volumes (e.g., in a syringe) with morphine, pethidine, promethazine, or hydroxyzine, as this will cause ketorolac to precipitate.
