Therapeutic Action
Vasodilator, antianginal
Extended-release formulation of isosorbide-5-mononitrate, which is released gradually over a 10-hour period.
Isosorbide mononitrate is an active metabolite of isosorbide dinitrate.
Indications
Long-term prophylaxis of angina pectoris due to coronary artery disease.
Long-term treatment of left-sided or global chronic heart failure in patients with intolerance to angiotensin-converting enzyme (ACE) inhibitors (e.g. enalapril)
Not indicated for the relief of acute angina attacks; for an acute attacks, use sublingual glyceril trinitrate tablets.
Instructions for use
Tablets should be swallowed whole, with half a glass of water. Do not chew or crush the tablets.
Posology (for adult only): start with 30 mg (half tablet) once daily in the morning for 4 days to minimise headaches side effect, then increase to 60 mg (one tablet) once daily in the morning. If needed, the dose may be increased to 120 mg (2 tablets) once daily in the morning.
Compared to 5 mg immediate-release isosorbide taken multiple times a day, the prolonged-release formulation reduces the risk of tolerance and increases patient compliance.
Precautions for Use
To be used in adults only.
Contraindications:
- Hypotension
- Obstructive myocardiopathy (e.g., aortic or mitral stenosis, constrictive pericarditis)
- Intracranial hypertension
- Neurologic injury
- Severe anaemia
Most common side effects: headache, dizziness, fatigue, nausea, tachycardia.
